The movement record form-A is required when you want to move out from one district to another district within the province of Punjab. This form can be used for one and more persons of a family. But it must be verified by the concerned district Deputy Commissioner office.
Following are the important steps to keep in mind while filling of movement record proforma:
1). Enter the name of Quarantine Centre including district where you spend 14 days and result is negative
2). Name, Percentage, CNIC and telephone number mentioned in the form
3). Memo No including Office stamp
