Culture, Tourism, Antiquities & Archives Department, Govt of Sindh has issued the official announcement again in the DAWN newspaper regarding the registration Proforma for Artist Health Insurance Card 2024-25. Sughars, Poets and of other relevant artist is required on following sample proforma throughout Sindh for issuance of health card to the most deserving artist and send the same to Directorate General Culture Sindh, Uaqat Memorial Library, Stadium road Karachi.
In pursuance of Meeting of Management Board for Endowment & Stipend Funds for poor & needy writers, poets and Artist held at Pakistan Institute of Tourism & Hotel Management and decision taken by the board, the complete bio data/profile of singer, musician, artisan, instrumentalist.
Health Insurance Card 2024 Eligibility Criteria
→ Singers, composers, musicians, folk poets (Sughar), writers (poets, story writers, novelists, critics, travelogue writers, researchers) dancers and artists (painters, calligraphers, sculptors) can fill this form.
→ Apart from them, skilled people involved in handicrafts such as jundi, kashi and needle thread can also fill this form.
→ The age of the candidate has been fixed as minimum 45 years and maximum 65 years.
→ Government employees and pensioners (except grade one to five) are not allowed to apply for this card.
→ Copy of CNIC and other important documents shall be provided along with this form.
→ It is a prerequisite for the writer and fork poet (Sughar) to be the owner of the book and at least one book must be included with the form.
→ In the case of an artist, it is necessary to include ten years of experience and copies of awards and certificates for performances should be attached.
→ Artists charging more than Rs. 50,000 as performance fees are not eligible to receive the health card.
→ This form can only be verified by members of the Management Board for Endowment Fund, whose names are as follows:
