Chief Executive Officer (DEA) Rahim Yar Khan has issued the official Notification letter on 11th February, 2020 in connection with show cause notice on anti dengue visits is zero.
WHEREAS, the undersigned as Competent Authority, under the Contract Policy in due consideration of the facts of this case is of the view that there are sufficient grounds to proceed against you on the following charges:
1. That as reported by the DHO, Rahimyarkhan vide No. 3654-60/CDC dated 06.02.2020, you are dormant android mobile user regarding Anti Dengue Activities and according to the said report, your entry regarding Anti Dengue Visits is Zero as a single visit I entry did not enter by you which amounts to “Inefficiency” on your part;
AND WHEREAS, the undersigned is of the opinion that it is not necessary to hold a further enquiry into the matter in view of the provisions contained in contract policy.
NOW THEREFORE, you are hereby called upon to show cause in writing on mentioned below date as to why one or more penalties as prescribed in contract policy should not be imposed upon you.
Your replies to the show cause notice should reach the undersigned within seven days, failing which it shall be presumed that you have no defense to offer.
In case you desire to consult any record, on which the aforesaid charges are based or is relevant to the aforesaid charges, you may do so with prior arrangement with the undersigned within three days of the receipt of this notice.
You are also directed to appear before the undersigned on 20-02-2020 at 3:30 PM for submission of your written defense with defensive documents/proves if you failed to avail the opportunity of Personal Hearing, the ex-parte action / decision will be taken against you under “Contract Policy”.
