TEVTA Notification Guidelines for Salary Disbursement Who Dies in Services

Government  of the Punjab, Technical Education & Vocational Training Authority (TEVTA) has issued the official Notification letter on 14th November, 2019 in connection with guidelines for salary disbursement on account of OSD posts to the family of a employee who dies in services.

Reference to TEVTA’s notification no. TEVTA / Bud / F. Assistance / 2018-19 dated 2nd May, 2018 and clarifications issued by Government of the Punjab, Finance Department’s letters no. FD.SR-I/3-10/2004 (P) dated 3rd October, 2017, no. FD.SR-1/3-3/2017 dated 29th November, 2017 and no. U.O No.FD-SR-1/3-4/2018 (180601/23199) dated 24th September, 2018.

It is clarified that TEVTA Account Section will process the case of pay fixation of deceased family salary after creation of OSD posts. The field offices will forward cases to TEVTA Head Office after observing the below mentioned documents and clarifications;


Documents Provided by Widow/Widower/Legal Heirs


  1. Applications from widow’s /widower/ Legal heirs forwarded by the head of institutes.
  2. Attested copy or CNIC or deceased employee.
  3. Attested copy of CNIC of widows / widower.
  4. Attested copy of Succession Certificate.
  5. Attested copy of list of Family Members from NADRA.
  6. Attested copies of CNICs of Family Members (Babies)
  7. Attested copy of Death Notification.
  8. Attested copy of Death Registration Certificate issued by the Secretary Union Council, Government of Punjab
  9. Affidavit of only widow/ widows/ widower.
  10. Copy or Guardian Certificate duly issued by court of law (if any)
  11. Attested copy of No Re-marriage Certificate from widows / widower.
  12. Attested copy of Nikah Nama.
  13. The detail of bank account in the Bank of Punjab only.

Documents Provided by Principal/HOI/District Manager/Zonal Manager/Service Center Office


  1. Copy or order or OSD post.
  2. Affidavit of recovery or Government Dues.
  3. Attested copy of last salary slip.
  4. Attested copy of last pay certificate.
  5. Attested copy of service book / Service Statement attested by the Head of institutions/DDO.
